    Enter your dog's medical information in the blank form above. We recommend that you also list multiple phone numbers along with your name if you have room.

    Our tags will fit up to 4 lines of information.

    The top line is longer and will fit 20 letters. Lines 2, 3, and 4 are shorter and fit only 18.

    Instructions for attaching name plates using rivets:

    Most of our collars are pre-punched for ID plates. If your collar is not, or you are adding a second ID plate:
    1. Use the ID plate to mark where it will be attached to the collar
    2. For leather collars, use a leather punch tool, ice pick, or hammer and nail to make the holes for the rivets
    3. For nylon collars, the holes should be melted using a soldering iron or a heated nail or ice pick
    Once you have your holes punched:
    1. Line the plate up on top of the holes and put the stud part of the rivet through from the bottom
    2. Put the crown over the stud and hammer it closed on a hard surface (concrete or steel)
    3. You should drive the 2 pieces together until the upper surface flattens out
    4. Your I.D. tag is now attached!
